By checking the above checkbox I hereby give my consent for my child to participate in all SportStrong Activities. I hereby release and discharge, indemnify and hold harmless SportStrong Youth Sports Foundation and its, members, officers, agents, employees and any other person or entities acting on their behalf against all claims demands and causes of action relating to injury, disability, death or other harm to person property or both, arising from participation. I have been informed and am aware of and assume all risks and incidental to and inherent in the game of basketball, including specifically, but not exclusively, the danger of being injured by running, contact with other athletes and waive all claims against SportStrong Youth Sports Foundation and the aforementioned entities and individuals. I understand that I am solely responsible for any costs arising out of any bodily injury, property damage, medical evacuation expense, or rescue expenses sustained through my child’s participation in SportStrong Youth Sports Foundation <
